Oral cavernous hemangioma: a rare case report and review of literature

OBJECTIVES: The aim is to report a case of buccal cavernous hemangioma, diagnosed on the basis of clinical, instrumental and histopathological findings and treated surgically.

MATERIALS AND METHODS: A 20-year-old woman presented with a swelling, involving 2/3 of the left anterior part of the buccal mucosa, and reported for about 10 years. There were no associated characteristics of pain, fever, difficulty in phonation and swallowing. Intraoral examination revealed a single large intramural and non mobile neoformation on the left anterior part of the buccal mucosa. Magnetic resonance imaging and ecodoppler method were employed for diagnosis and volumetric  analysis.

RESULTS AND CONCLUSIONS: Depending on the clinical and instrumental features, a provisional diagnosis of hemangioma was made. The surgical approach was employed. The excised mass was about 3.5x2x1.5 cm, showing macroscopically nodular areas with an adipose, spongy and hemorrhagic appearance. Histopathological examination confirmed the definitive diagnosis of cavernous hemangioma.

CLINICAL SIGNIFICANCE: The treatment of oral cavernous hemangioma is based on several factors such as size, position, accessibility, depth of invasion, patient age and aesthetic aspects. Many non-surgical treatments have been suggested as watchand- wait, cryotherapy, laser therapy, isolated embolization, sclerosing agents or steroid injection. However, the results of these methods are controversial, so they are recommended only when surgical treatment is contraindicated.
