Orthodontic-Periodontic Synergy: managing bimaxillary protrusion with canine ectopia

OBJECTIVES: Bimaxillary protrusion is a common aesthetic concern driving patients for orthodontic therapy. However, its management becomes significantly
more complex when combined with ectopic eruption and compromised periodontium. This case report explores the potential for orthodontic treatment to achieve functional and aesthetic rehabilitation in such challenging cases, potentially highlighting the importance of interdisciplinary synergy between orthodontics and periodontics.

MATERIAL AND METHODS: A 21‐year‐old male patient with significant habit history was treated for bimaxillary protrusion associated with canine ectopia, mutilated dentition, chronic irreversible pulpitis and localized periodontitis.

RESULTS: Employing modified orthodontic biomechanics in conjunction with periodontal therapy to address mucogingival deficiencies, this holistic approach resulted in a multifaceted improvement for the patient, enhancing facial profile, corrected protrusion, resolution of the ectopic eruption, establishment of occlusion and periodontal stability.

CONCLUSIONS: This case report highlights the critical role of comprehensive diagnosis to identify coexisting problems and formulate a treatment plan tailored to the patient’s specific needs for optimal outcomes.

CLINICAL SIGNIFICANCE: Orthodontic-periodontal integration enhances aesthetics, occlusal stability, and periodontal health in complex cases through tailored multidisciplinary care.
